Monday-Friday 0730-1630 Active Duty, ADFM, DOD (under certain circumstances) and Retired Service Members and their families that are enrolled in Tricare Prime.

Services: Provides comprehensive vision examinations for patients age 5 years and older with priority given to AD service members. The optometrists prescribe glasses and conduct eye health evaluations. The clinic order glasses and required optical devices for all AD service members. Patients may make a routine vision examination without a referral.

    • (Initial Class 1 Flight Physical vision section by appointment due to the amount of testing and dilation)

    • 0730-1100 & 1300-1600 -- AD and Retired military with current glasses prescription ordering glasses (less than a year).

Services: The Strong Heart Warrior Post-Traumatic Stress Support Group is sponsored by the Regimental Memorial Chapel to support the resiliency and healing for any service member or veteran. Services: The Women’s Health Clinic specializes in services for women of all ages and through all phases of life. Key services include annual well woman visits, referrals for additional services needed, birth control counseling with options for prescriptions, devices, injections or surgery, sexually transmitted disease testing and treatment, infertility evaluations, management of menstrual irregularities, menopausal transitions and hormonal issues, breast issues, gynecologic infection treatment, and other specific gynecological concerns. Outpatient surgery for the diagnosis and treatment of various conditions is also offered at this facility.

Transition Services: The Army Career and Alumni Program (ACAP) delivers a world-class transition program for America’s Army that ensures that all eligible transitioners have the knowledge, skills and self-confidence necessary to be competitive and successful in the global workforce. ACAP helps transitioners to make informed career decisions through benefits counseling and employment assistance. ACAP is responsible for delivering both transition assistance and employment assistance services.
Pre-Separation

ACAP provides a wide range of services designed to make transition an easy and successful experience. The key to transition success is knowledge of those services and a firm understanding of how to use them. All tansitioning Soldiers are required to visit the ACAP Center prior to departing the Army. Military personnel will receive services by attending a mandatory pre-separation briefing no less than 90 days prior to separation. During the briefing, Soldiers learn about their benefits and rights, as well as the services that are available. During the briefing Soldiers are introduced to the DD form 2648 “Pre-Separation Counseling Checklist” and assisted in its completion. This form helps Soldiers to identify their needs and develop a personal plan for securing the help to meet those needs.

Services: The Soldiers’ Medical Evaluation Board (MEB) Counsel Office at Fort Eustis assists Soldiers undergoing the MEB process at Fort Eustis and also reviews the findings of the Informal Physical Evaluation Board (IPEB) with Soldiers. Furthermore, through the addition of the Soldier's Medical Evaluation Board Counsel, Soldiers now have access to an attorney to assist them in understanding the medical board process. The Fort Eustis MEBC office also serves Soldiers stationed at Fort Lee, Joint Expeditionary Base Little Creek-Fort Story, all other detached Army units in the Hampton Roads area and the Army Reservists and National Guardsmen assigned to Virginia's Community-Based Warrior Transition Unit.

Services: The Passport Office processes passport applications for active duty military, DOD civilians, and civilians on official orders for overseas assignment (TDY or PCS). Once individuals have been notified that they are on levy for overseas travel, they are advised to proceed to the passport office, where the following process begins:

Individuals are interviewed to determine whether their destination requires a passport and/or visa


Family members’ citizenship is verified. If they are not US citizens, they are advised to ensure their foreign passports are up-to-date. If not, they must have them renewed through their respective embassy.
In cases of stateless aliens, the US Immigration & Naturalization Office is the agency responsible for this individual’s travel documents
If the military sponsor has selected an accompanied, command-sponsored tour, then passport applications are issued for the completion by all Family members

Individuals are then advised of legal documents necessary for obtaining a passport (birth certificate, naturalization certificates, reports of birth abroad, adoption papers and/or previously issued passports, as applicable). Persons without one or more of the necessary legal documents are informed of how they can obtain them.

No-fee passport photos are taken at the ID Card Section, building 650. Persons requiring photos for passports must see the passport agent to obtain a “photo request slip.” Passport photos are not taken without a slip.
Individuals on orders for overseas travel should check with the Passport Agent as soon as possible to determine whether a passport and/or visa is necessary for the destination for which they have been assigned.
