Video Request Form

Loan Procedures

The TMSHP video library is for you; the small mine operator and your employees. These videos can be used in the safety training programs you provide your employees.


How to Borrow

You can request up to three videos at a time and you can keep them for up to one week. The due date shown on the Video Request Form is the date the video is due in Austin. The Video Request Form will be mailed with the video. The information on this form will help us improve our service to you.


Videos can be borrowed once a signed, written request is received by the TMSHP. You can mail or fax your request. Videos are loaned on a first come, first served basis – there are limited quantities. An early request will increase the chance of availability. Please allow us at least seven days to get your requested videos to you.

How to Return

Be Kind and Rewind: please rewind videos. Re-package the videos and mail back to us at the address below. Please return in the manner it was sent to you, with some form of tracking information.
